Registered Nurse (RN) - Care Manager

About the role

The Registered Nurse (RN) Care Manager coordinates patient care across inpatient, emergency, and outpatient settings, with a focus on geriatric patients.

Responsibilities

• Conduct comprehensive patient assessments
• Facilitate multidisciplinary care planning
• Manage discharge planning
• Support quality outcomes: reduce length‑of‑stay, prevent readmissions, enhance patient satisfaction
• Collaborate with healthcare teams
• Perform utilization reviews
• Ensure proper documentation
• Provide patient education
• Coordinate transitional planning
• Participate in performance improvement activities

Qualifications

• BSN or MSN degree
• Active RN license in New York
• Certifications such as Patient Review Instrument (PRI)

Preferred Experience

• 1–3 years of clinical experience in Emergency, Critical Care, or Medical‑Surgical settings
• Case management background
• Familiarity with Milliman/InterQual criteria

Skills

• Strong computer skills
• Knowledge of healthcare regulations, payer issues, and state/federal programs
