Registered Nurse (RN) - Care Manager

We are seeking a Licensed Registered Nurse (RN) Care Manager to collaborate with healthcare teams in assessing patient needs, coordinating care, and facilitating safe discharges across inpatient, emergency, and outpatient settings. Key Responsibilities Include Conducting comprehensive patient assessments and coordinating multidisciplinary care.Managing transitional planning, including patient education, arranging after-care services, and involving payers.Supporting department goals such as reducing length-of-stay, preventing readmissions, minimizing denials, and enhancing patient satisfaction.Performing utilization reviews, clinical documentation, and participating in performance improvement activities.Collaborating with healthcare professionals to evaluate patient needs and ensure safe discharges.Adhering to organizational policies and performing additional duties as assigned. Qualifications & Skills BSN or MSN required; RN license valid in the state.1–3 years of clinical nursing experience preferred, especially in Emergency, Critical Care, or Medical-Surgical settings.Previous case management experience and familiarity with Milliman/InterQual criteria preferred.Knowledge of healthcare regulations, financial issues, and payer systems.Strong computer skills and relevant certifications such as PRI, NYS SCREEN, or Care Management credentials preferred. This role offers a salary range of $124,156 – $158,065 and involves day shifts with alternating weekends and holidays, focusing on improving patient outcomes and operational efficiency.
