Registered Nurse (RN) - Care Manager

The Registered Nurse (RN) Care Manager is responsible for assessing patient needs, coordinating multidisciplinary care, facilitating safe discharge planning, and supporting quality outcomes across inpatient, emergency, and outpatient settings. Key duties include conducting comprehensive initial assessments, managing care transitions, educating patients and caregivers, arranging after-care services, and collaborating with healthcare teams to reduce length of stay, prevent readmissions, and improve patient satisfaction. Qualifications include a BSN or MSN, an active New York State RN license, and certifications such as PRI. Prior experience in clinical nursing (preferably in Emergency, Critical Care, or Medical-Surgical) and case management is preferred. Skills required are strong computer proficiency, knowledge of healthcare financial and regulatory issues, and familiarity with payer systems and criteria like Milliman/InterQual. Ideal candidates are collaborative, detail-oriented, and committed to quality care. The role requires participation in performance improvement activities, utilization review, and effective communication within healthcare teams. Work schedule is Monday–Friday, 8-hour days, with alternating weekends and holidays. Salary ranges from $124,156 to $158,065.
