Registered Nurse (RN) - Care Manager

About the role

The Registered Nurse (RN) Care Manager is responsible for assessing patient needs, coordinating multidisciplinary care, facilitating safe discharge planning, and supporting quality outcomes across inpatient, emergency, and outpatient settings.

Responsibilities

• Conducting comprehensive initial assessments
• Managing care transitions
• Educating patients and caregivers
• Arranging after-care services
• Collaborating with healthcare teams to reduce length of stay, prevent readmissions, and improve patient satisfaction
• Participating in performance improvement activities
• Utilization review
• Effective communication within healthcare teams

Qualifications

• BSN or MSN
• Active New York State RN license
• Certifications such as PRI
• Prior experience in clinical nursing (preferably in Emergency, Critical Care, or Medical‑Surgical) and case management is preferred

Skills

• Strong computer proficiency
• Knowledge of healthcare financial and regulatory issues
• Familiarity with payer systems and criteria like Milliman/InterQual
• Collaborative, detail‑oriented, and committed to quality care

Pay

Salary ranges from $124,156 to $158,065.

Schedule

Work schedule is Monday–Friday, 8‑hour days, with alternating weekends and holidays.
