About the role
The Registered Nurse (RN) Care Manager coordinates patient care across inpatient, emergency, and outpatient settings, focusing on geriatric populations.
Responsibilities
- Conduct comprehensive assessments
- Facilitate multidisciplinary care
- Manage discharge planning
- Support quality outcomes such as reducing length-of-stay and readmissions
- Collaborate with healthcare teams
- Ensure compliance with policies
- Complete documentation
- Participate in performance improvement activities
- Review utilization
- Support discharge processes
- Advocate for patient education and appropriate post‑care arrangements
Qualifications
- BSN or MSN
- NYS RN licensure
- PRI certification within 90 days
- Care management certification (e.g., CCM or CMC) preferred
Skills
- 1–3 years of clinical experience, preferably in ED, Critical Care, or Medical‑Surgical
- Prior case management experience
- Strong computer skills
- Knowledge of healthcare regulations, financials, and payer issues
