Registered Nurse (RN) - Care Manager

We are seeking a Registered Nurse (RN) Care Manager to coordinate patient care across various settings, ensuring quality outcomes and patient satisfaction. Responsibilities include resource management for cost-effective care transitions, health risk assessments, health promotion, and clinical outcome studies. The RN educates payers, providers, and the public about healthcare trends and initiatives to enhance care delivery and optimize resource utilization.
