Registered Nurse (RN) - Care Manager

About the role

We are seeking a Registered Nurse (RN) Care Manager to coordinate patient care across various settings, ensuring quality outcomes and patient satisfaction. The role involves assessing, planning, monitoring, and evaluating patients' care plans in collaboration with healthcare teams.

Responsibilities

• Resource management for cost-effective care transitions
• Health risk assessments
• Health promotion
• Clinical outcome studies
• Educating payers, providers, and the public about healthcare trends and initiatives to enhance care delivery and optimize resource utilization

Qualifications

• Associate’s in Applied Science in Nursing (required)
• Bachelor’s in Nursing (preferred)
• Minimum of two (2) years of experience in an acute or tertiary care facility (required); five (5) years (preferred)
• NY State Registered Nurse license (required)
