About the role
Registered Nurse (RN) Care Manager responsible for coordinating patient care across various settings.
Responsibilities
- Assess, plan, monitor, and evaluate patient care plans in collaboration with healthcare teams.
- Manage resources to ensure cost-effective transitions to appropriate care levels while maintaining quality outcomes and patient satisfaction.
- Monitor care processes, collect data for quality improvement, and collaborate with physicians and healthcare staff.
- Educate payers, providers, and the public on healthcare trends and initiatives to enhance care delivery and resource utilization.
Qualifications & Skills
- Associate's in Applied Science in Nursing (minimum); Bachelor's of Science in Nursing (preferred).
- Minimum of two years of experience in an acute/tertiary care facility; five years preferred.
- Valid NYS Registered Nurse license.
Work Environment & Additional Details
- Roles are performed in office or hospital units with adherence to safety protocols, utilizing appropriate protective equipment.
- Serves patients of all ages with HIPAA access levels to patient information.
