Registered Nurse (RN) - Care Management

Role Overview: The Case Manager (Acute Care) is responsible for coordinating clinical and financial systems to ensure positive health outcomes. Key Responsibilities Ensure chart/EMR accuracy aligns with InterQual review or CMS guidelines through collaboration with Utilization Review RN.Assess and re-evaluate the patient's care plan promptly upon admission.Communicate with physicians for additional information and collaborate with physician advisors when necessary.Meet with patients and families within 48 hours of admission to assess needs, provide support, and facilitate discharge planning.Promote safe transition practices to reduce readmissions and ensure smooth post-acute care transitions. Qualifications & Skills Registered Nurse (RN) licensed in the state; BSN preferred, with a requirement to obtain within 5 years if not already held.Minimum of two years hospital experience preferred; related experience in home care or payer sectors considered.Strong autonomous clinical skills, problem-solving, collaboration, and critical thinking abilities.Proficient in computer use, data analysis, and effective verbal and written communication.Self-directed with decision-making capabilities and a commitment to continuous improvement.
