About the role
Seeking a Care Coordination RN Senior to assess, plan, and coordinate care for complex patients during acute hospital stays, primarily in the Emergency Department.
Responsibilities
- Conducting transitional care planning
- Evaluating psychosocial and functional needs
- Collaborating with physicians and interdisciplinary teams
- Facilitating timely discharge and post-acute referrals
- Managing discharge processes
- Documenting assessments and care plans
- Mentoring new staff and students
Qualifications
- Minimum of 3 years of acute care nursing experience
- 2 years in hospital-based care coordination or case management
Preferred qualifications
- Bachelor's in Nursing
- BLS certification
- RN licensure
