About the Role

A Senior Care Coordinator RN assesses, plans, and facilitates patient discharge and care progression in an acute care setting, with a focus on the Emergency Department.

Responsibilities

  • Conduct comprehensive assessments of patients' clinical, psychosocial, and resource needs.
  • Collaborate with physicians, care teams, patients, and families to develop and implement individualized discharge plans.
  • Manage barriers to discharge, coordinate post-acute care referrals, and ensure timely progression of treatment.
  • Participate in interdisciplinary rounds and patient conferences to streamline care and promote efficient resource utilization.
  • Mentor new staff and precept students to foster team development.
  • Document assessments, care plans, and interactions accurately in medical records.
  • Support professional development and adherence to healthcare regulations and policies.

Requirements

  • Minimum of 3 years of clinical experience in an acute care hospital.
  • 2 years of experience in hospital-based care coordination.
  • Current RN license with BLS certification; multi-state licensure preferred.

Skills

  • Strong communication, organizational, and problem-solving skills.
  • Knowledge of healthcare regulations, community resources, and care management principles.
  • Ability to work collaboratively, handle stress, and adapt to a fast-paced environment.
