About the Role
A Senior Care Coordinator RN assesses, plans, and facilitates patient discharge and care progression in an acute care setting, with a focus on the Emergency Department.
Responsibilities
- Conduct comprehensive assessments of patients' clinical, psychosocial, and resource needs.
- Collaborate with physicians, care teams, patients, and families to develop and implement individualized discharge plans.
- Manage barriers to discharge, coordinate post-acute care referrals, and ensure timely progression of treatment.
- Participate in interdisciplinary rounds and patient conferences to streamline care and promote efficient resource utilization.
- Mentor new staff and precept students to foster team development.
- Document assessments, care plans, and interactions accurately in medical records.
- Support professional development and adherence to healthcare regulations and policies.
Requirements
- Minimum of 3 years of clinical experience in an acute care hospital.
- 2 years of experience in hospital-based care coordination.
- Current RN license with BLS certification; multi-state licensure preferred.
