Registered Nurse - Acute Care - Care Coordination - Full Time Days

About the Role

The RN Complex Care Coordinator assesses and manages complex patient discharge needs, coordinating care across settings, and engaging patients and families to ensure safe and timely transitions. They serve as an expert resource for clinical, psychosocial, and social determinants of health issues, providing guidance on community resources, end-of-life planning, and addressing barriers to discharge.

Responsibilities

• Conduct comprehensive assessments and develop individualized discharge plans
• Facilitate interdisciplinary team collaboration and advocate for patient-centered care
• Mentor new staff and support performance improvement initiatives
• Ensure accurate documentation in the electronic health record
• Implement discharge planning and assess psychosocial risks
• Collaborate with care teams and facilitate referrals to community resources
• Support professional development and mentorship efforts

Requirements

• Active RN license (single or multi-state)
• BLS certification
• Minimum 3 years of healthcare experience, including 2 years in care coordination within an acute setting
• Strong communication, assessment, organizational, and problem-solving skills
• Knowledge of healthcare regulations and community resources
