About the role
The RN Complex Care Coordinator plays a vital role in assessing and managing the care of complex patients with high risk of readmission or extended stays.
Responsibilities
- Conduct comprehensive clinical and psychosocial assessments.
- Develop and implement discharge plans.
- Coordinate care across the continuum.
- Partner with interdisciplinary teams.
- Provide resource guidance and address social determinants of health.
- Support end-of-life planning and community services.
- Mentor new staff and precept students.
- Support quality improvement initiatives.
Requirements
- Nursing degree (Associate, Diploma, or Bachelor’s).
- Active RN licensure.
- Certifications such as BLS.
- At least 3 years of healthcare experience.
- Minimum 2 years in care coordination in an acute setting.
