About the role

The RN Complex Care Coordinator plays a vital role in assessing and managing the care of complex patients with high risk of readmission or extended stays.

Responsibilities

• Conduct comprehensive clinical and psychosocial assessments.
• Develop and implement discharge plans.
• Coordinate care across the continuum.
• Partner with interdisciplinary teams.
• Provide resource guidance and address social determinants of health.
• Support end-of-life planning and community services.
• Mentor new staff and precept students.
• Support quality improvement initiatives.

Requirements

• Nursing degree (Associate, Diploma, or Bachelor’s).
• Active RN licensure.
• Certifications such as BLS.
• At least 3 years of healthcare experience.
• Minimum 2 years in care coordination in an acute setting.
