Role Overview
The RN Complex Care Coordinator assesses and manages care for complex patients, ensuring safe and effective discharge planning across the continuum.
Responsibilities
- Conducts comprehensive clinical and psychosocial assessments
- Develops individualized discharge plans
- Collaborates with interdisciplinary teams to coordinate community resources, post-acute care, and social services
- Provides guidance on psychosocial, legal, and resource needs, including end-of-life planning
- Acts as a resource for care team members and mentors new staff
- Facilitates hospital meetings, advocates for patients, and supports community agency negotiations
- Maintains accurate documentation
- Participates in professional development and protocol development
Qualifications & Skills
- Minimum 3 years healthcare experience, with at least 2 years in care coordination in an acute care setting
- Strong clinical, social, and resource management expertise
- Excellent communication, assessment, and mentoring abilities
- Ability to handle complex psychosocial and discharge challenges, and work collaboratively to optimize patient outcomes
