Registered Nurse - Acute Care - Care Coordination (Full-Time)

About the role

The Senior Care Coordinator RN assesses patient needs, manages care progression, and plans discharge for complex cases in an acute care hospital setting, primarily in the Emergency Department.

Responsibilities

  • Collaborates with physicians and care teams to facilitate timely care and discharge, addressing barriers and ensuring effective transition planning.
  • Conducts psychosocial assessments and coordinates post-acute referrals.
  • Documents care plans accurately and mentors new staff.
  • Participates in interdisciplinary rounds and patient conferences, advocating for patient needs and resource utilization.
  • Precepts new team members and engages in professional development.
  • Adheres to hospital policies and procedures.

Requirements

  • Minimum 3 years of acute care nursing experience.
  • 2 years of experience in hospital-based care coordination.
  • Active RN licensure and BLS certification.
  • Associate’s or Bachelor’s degree in Nursing or related field.

Skills

  • Excellent communication and clinical assessment abilities.
  • Strong problem-solving, adaptability, and teamwork.
  • Knowledge of healthcare regulations and community resources.
