Registered Nurse (RN) - Acute Care - Care Coordinator

Role: RN Care Coordinator responsible for assessing, planning, and managing patient care transitions in a hospital setting. Responsibilities Conduct psychosocial and functional assessments, develop individualized discharge plans, and facilitate care progression.Collaborate with physicians, interdisciplinary teams, patients, and families to ensure seamless care coordination and discharge readiness.Manage discharge processes, address barriers, and coordinate post-acute referrals to ensure timely patient transitions.Identify delays in care, resolve obstacles, and communicate effectively to promote efficient patient throughput.Maintain accurate, timely documentation of assessments, care plans, and interactions.Support professional development, serve as a mentor when appropriate, and adhere to organizational policies. Qualifications & Skills Associate's Degree in Nursing and valid RN license (multi-state or single-state as applicable).Minimum of 1-year experience in acute care nursing.Strong communication, assessment, organizational, and problem-solving skills.Knowledge of healthcare regulations, community resources, and care coordination processes.Ability to work collaboratively with healthcare teams and handle stressful situations professionally.
