Role Overview

The RN Care Coordinator Sr. is responsible for assessing transitional care needs, coordinating care across the continuum, and engaging patients and families to ensure optimal discharge planning. They collaborate with physicians and interdisciplinary teams to facilitate care progression, identify barriers, and promote timely discharges. The role involves managing discharge processes, developing individualized plans, and ensuring documentation accuracy. Additionally, they mentor new staff, participate in education, and support departmental goals. The position requires strong clinical expertise, experience in hospital settings, and case management skills.

Qualifications & Skills

• Associates or Diploma in Nursing required; Bachelors preferred
• Minimum of 3 years hospital RN experience
• At least 2 years in hospital-based case management
• Current Basic Life Support certification
• Ability to collaborate effectively, assess psychosocial factors, and manage discharge planning
