Registered Nurse - Rehab - Full-Time 8-Hour Day Shift

About the Role

This position involves assessing residents' health, educational, and environmental needs, developing and updating personalized care plans through interdisciplinary collaboration, and ensuring safe and timely delivery of nursing interventions. The nurse communicates effectively with team members, residents, and families, documents all assessments and interventions accurately, and maintains awareness of residents' changing conditions. Critical thinking, crisis management, and adherence to best nursing practices are essential.

Responsibilities

• Assess and document residents’ health status and needs.
• Develop, review, and revise care plans collaboratively.
• Implement nursing interventions, including medication and treatments.
• Communicate changes and coordinate with the interdisciplinary team.
• Monitor care delivery and address problem areas using QAPI processes.
• Respond to residents’ needs, assist with ADLs, and answer call lights.
• Maintain compliance with regulatory and documentation standards.
• Participate in ongoing education to stay current with nursing practices.

Requirements

• Registered nurse license in the relevant state.
• Graduate from an accredited nursing program.
• CPR/AED certification (or willingness to obtain).
• Critical thinking, excellent communication, and organizational skills.
• Physical ability to stand, walk, lift, and assist with residents and equipment.
• Manual dexterity and visual acuity.

Qualifications

• Long-term care experience preferred.
