Part-Time Registered Nurse (RN) Rehab Nurse - 12-Hour Day Shift

About the Role

This position involves providing comprehensive nursing care to residents, including assessing health status, developing and updating care plans through an interdisciplinary approach, and ensuring timely and accurate documentation of assessments and interventions.

Responsibilities

• Assess residents' health, educational, and environmental needs upon admission and regularly thereafter.
• Communicate significant changes to the interdisciplinary team and document all assessments.
• Develop, review, and revise personalized care plans involving residents, families, and healthcare professionals.
• Implement nursing interventions such as medication administration, treatments, and IV therapies safely and accurately.
• Monitor residents closely, utilize critical thinking for changes in condition, and notify physicians using SBARR protocol.
• Handle crisis situations professionally, address care issues through QAPI processes, and ensure effective care delivery.
• Assist residents with activities of daily living and respond to call lights promptly.
• Complete ongoing education to stay current with nursing practices.

Requirements

• Registered nurse license from an accredited school and current state licensure.
• CPR/AED certification or willingness to obtain within six months.
• Critical thinking, excellent communication, and organizational skills.
• Physical ability to stand, walk, lift heavy objects, and assist with patient transfers safely.
• Manual dexterity, visual acuity, and ability to perform electronic and written documentation.

Preferred Qualifications

• Experience in a long-term care setting.
