About the Role

Provides comprehensive nursing care in an inpatient acute rehab setting, acting as the central coordinator for patient management and interdisciplinary collaboration.

Responsibilities

• Implement and adjust care plans
• Interpret and execute physician and nurse orders
• Maintain vigilant patient observation
• Document accurately in the EHR
• Communicate significant changes promptly
• Assist with discharge planning by coordinating post-acute resources
• Educate patients and families for safe transitions

Requirements

• Valid RN license from the Wisconsin Board of Nursing or applicable multi-state license
• BLS certification within one month of hire
• Diploma from an accredited nursing program
