Responsibilities

• Acting as the central care coordinator, facilitating interdisciplinary collaboration for optimal patient outcomes.
• Implementing, monitoring, and adjusting individualized care plans based on clinical findings and team input.
• Interpreting, documenting, and executing physician and nursing orders accurately.
• Conducting clinical observations, maintaining meticulous documentation, and communicating significant changes promptly.
• Proactively planning discharges by collaborating with patients, families, and healthcare team members to ensure safe transitions.

Qualifications

• Current RN licensure from the Wisconsin Board of Nursing or applicable multi-state license.
• Basic Life Support (BLS) certification within 30 days of hire.
• Diploma from an accredited nursing program or equivalent professional licensure.
