About the role
The role is based in Mequon, WI at Sacred Heart Rehab in the Acute Rehab department, working night shifts (7 pm to 7:30 am, 24 hours/week).
Responsibilities
- Care coordination
- Managing care plans
- Interpreting and executing physician orders
- Maintaining vigilant patient observation and documentation
- Proactive discharge planning
- Effective communication with the healthcare team to ensure seamless patient transitions
Requirements
- Licensed as a Registered Nurse in Wisconsin or a multi-state/Compact license
- BLS certification within 30 days of hire
Qualifications
- Experience in acute care settings preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
