Registered Nurse Rehab

Responsibilities

• Care coordination and interdisciplinary collaboration to optimize patient outcomes.
• Implementing, monitoring, and adjusting individualized care plans based on clinical findings and team input.
• Accurately interpreting, documenting, and executing physician and nursing orders.
• Maintaining vigilant patient assessments, documenting findings meticulously, and communicating significant changes promptly.
• Proactively planning discharges, coordinating post-care services, and educating patients and families for smooth transitions.

Qualifications

• Current RN licensure from the Wisconsin Board of Nursing or a multi-state license.
• Basic Life Support (BLS) certification within 30 days of hire.
• Diploma from an accredited nursing program or equivalent professional licensure.
