About the role
Care Coordination: Acts as the central coordinator, ensuring seamless management and interdisciplinary collaboration (physicians, specialists, therapists, social workers, ancillary staff) for optimal patient outcomes.
Care Plan Management: Implements, continuously monitors, and dynamically adjusts the prescribed patient care plan based on clinical findings, patient progress, and team input, requiring critical thinking and timely decision-making.
Order Management: Precisely interprets, documents, and executes all authorized physician and nursing orders, including medications, tests, treatments, and procedures.
Clinical Observation, Documentation, and Communication: Maintains constant vigilance, systematically assesses the patient, meticulously documents findings and treatments in the EHR, and immediately communicates all significant changes to the physician and healthcare team.
Discharge Planning: Proactively assesses discharge needs from admission, collaborating with the patient, family, social work, case management, and physicians to arrange post-acute resources and provide education for a safe transition of care.

Requirements
- Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
