Registered Nurse - Night On Call - Community Hospice

About the role

We are seeking a full-time On Call Visit RN to support clients in the home setting from 5pm to 8am. The Community Hospice team delivers care to patients in the community, providing skilled nursing care and support.

Responsibilities

• Deliver care to patients in the community
• Collaborate with the triage nurse team
• Provide admissions support during the first 5 hours of on-call shifts
• Be available for urgent calls during the remaining on-call hours

Requirements

• Associate's degree in nursing or higher
• Minimum of four years of experience as Community Hospice Nurse or minimum of five years of experience in home health nursing
• Certification in hospice and palliative care preferred
• Live within reasonable driving distance to our service area
• On-call: Required

Qualifications

• Current RN licensure recognized by the State of Vermont required
• Appropriate experience in specific clinical area
