Registered Nurse - Inpatient Hospice Discharge Coordinator

About the role

Park Nicollet is seeking a Hospice Coordinator to join their hospice team. This role is part of the "Partner for Good" initiative, aiming to enhance the care and experiences of patients and their families.

Responsibilities

• Ongoing physical, psychosocial, and emotional assessment of Hospice patients and families.
• Provide continued teaching and support to patients, families, and care staff.
• Ensure the plan of care is being carried out and revise it as needed.
• Aid in meeting patients' comfort care needs and promote a peaceful and comfortable death.

Requirements

• Bachelor’s degree in nursing.
• Current MN RN license.
• Demonstrated knowledge of age-related growth and development principles.
• Sensitivity and skill in assessment, communication, flexibility, health counseling, and documentation.
• Excellent written and oral communication skills.
• Committed to hospice philosophy and customer service.
• Knowledge of reimbursement regulations.
• Ability to problem solve and interface with primary customers.
• Knowledgeable about age-related growth and development principles.

Preferred Qualifications

• Previous supervisory experience.
• At least five years of hospice and intensive care nursing experience.
