About the Role

The Hospice RN Clinical Coordinator carries out administrative and management functions and oversight in the absence of the Director of Clinical Services. This role may also carry a caseload due to fluctuations in patient census and is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.

Responsibilities

  • Promotes a positive working relationship between team members, program leadership, agency, and corporate personnel.
  • Models desired behaviors including timeliness, punctuality, attendance, collaboration, open communication, and equitable treatment.
  • Attends and participates in staff meetings, educational programs, and community events as requested.
  • Promotes Hospice philosophy and communicates identified needs and potential solutions to leadership.
  • Assists with onboarding and provides mentorship to new staff.
  • Manages DME and medical supply orders, monitors supply closet for expiration dates, and ensures needed medications are available for admissions.
  • Submits requests for medical records, hospital inpatient charts, and processes provider notifications.
  • Answers incoming telephone calls, takes accurate messages, and transfers calls appropriately.
  • Assists with Interdisciplinary Team meetings, including agendas and coordination of updates.
  • Provides coverage for the Director of Clinical Services as needed.
  • Processes workflow for admissions, recertifications, and new physician orders.
  • Supports team scheduling to meet patient care plans, including HOPE, GAP, and HVLDL visits.
  • Serves as MUSE Champion to oversee changes in patient acuity and required daily visits.
  • Actively participates in Quality Assessment and Performance Improvement (QAPI) activities.
  • Participates in the development and update of patient care policies and emergency procedures.
  • Makes post-admissions calls if directed and documents in patient clinical records as required.
  • Participates in care delivery for patients, including admissions, recertification visits, routine visits, and discharge visits.
  • Manages care for a caseload of patients to support census changes.
  • Participates in after-hours shifts, including primary and back-up on-call, and performs patient admissions and visits as needed.
  • Makes visits to hospitals or customers for additional educational support.
  • Performs RN Case Manager responsibilities as needed and may carry a caseload due to patient census fluctuations.
  • Performs other duties as assigned.

Requirements

  • Active and unencumbered Registered Nurse License.
  • One (1) year of Hospice experience required.
  • Valid driver’s license and auto liability insurance.
  • Current CPR certification.

Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, and external parties.
  • Strong written and verbal communication skills.
  • Ability to understand, read, write, and speak English.
  • Articulates and embraces integrated healthcare at home philosophy.

Physical Demands and Work Environment

The demands of this role necessitate regular standing, walking, and manual dexterity, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
