Clinical Services Coordinator (RN) - Hospice Home Care

About the role

Provides clinical administrative support and clinical expertise to meet patient and staff needs. Functions as a liaison with physician practices, hospital case management, and other referral sources.

Responsibilities

• Analyzes data to prioritize multi-issue situations and supports appropriate actions based on patient health and welfare, patient/staff safety, and service effectiveness.
• Identifies barriers to situation or complaint resolution and follows up with appropriate resources or leadership to address issues in a timely manner.
• Reviews and supports the development of patient-specific clinical plans of care, ensuring effective communication with involved parties and supporting quality patient outcomes and satisfaction.
• Supports clinical staff in implementing identified plans, coordinating care delivery, and promoting health and safe outcomes.
• Evaluates progress toward attainment of outcomes through documentation review, interdisciplinary team meetings, and/or case conferences.
• Supports billing compliance and timeliness by maintaining current knowledge of payer requirements, billing criteria, state and federal regulations, completing CSC workflow timely, and identifying opportunities for appropriate reimbursement.
• Delegates elements of care to appropriate healthcare workers in accordance with legal or policy parameters.
• Collaborates with healthcare consumers, family, and others in the conduct of clinical practice.
• Utilizes appropriate resources to plan and provide clinical services that are safe, effective, and financially responsible.
• Participates in committees, shared governance, or other facility decision-making activities.

Physical Requirements

• Primary work environment is office-based with long periods of sitting and computer work.
• May require walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
• Must be able to lift and support up to 35 pounds when handling patients, medical equipment, and supplies in home or facility settings.
• Must speak English clearly and understandably.
• Intact senses of sight, hearing, smell, and touch; finger dexterity; critical thinking; concentration.
• Ability to respond quickly to changes in patient conditions.

Requirements

• Graduate from an accredited School of Nursing.
• BSN preferred.
• BLS certification required per policy guidelines.
• One year of experience in a related field required.
• Current RN license or temporary RN license as a Registered Nurse Petitioner in the state of work and residence.
• If declaring a National License Compact (NLC) state as primary state of residency, meet licensure requirements in home state.
• For Non-NLC states, current RN license or temporary RN license as a Registered Nurse Petitioner required in the state where the RN works.
• Must maintain a current state driver’s license and auto insurance.
• Additional education, training, certifications, or experience may be required within the department by the nurse leader.
