Hospice RN Clinical Coordinator
About the Role
The Hospice RN Clinical Coordinator carries out administrative and management functions and provides oversight in the absence of the Director of Clinical Services. This role may also manage a caseload due to fluctuations in patient census and is responsible for applying state and federal regulatory agency rules.
Responsibilities
- Promote a positive working relationship between team members, program leadership, agency, and corporate personnel.

- Attend and participate in staff meetings, educational programs, and community events as requested.
- Promote Hospice philosophy and communicate identified needs and potential solutions to leadership.
- Assist with onboarding and provide mentorship to new staff.
- Manage DME and medical supply orders, monitor supply closet for expiration dates, and advise leadership on trends.
- Oversee medication orders to ensure availability for admissions and obtain approval for out-of-formulary items.
- Submit requests for medical records and hospital inpatient charts, and process provider notifications.
- Answer incoming telephone/clinical calls, take accurate messages, and transfer calls appropriately.
- Assist with Interdisciplinary Team meetings, including agendas and coordination of updates.
- Provide coverage for the Director of Clinical Services as needed.
- Process HCHB workflow for admissions, recertifications, and new physician orders.
- Support team scheduling to meet patient care plans, including HOPE, GAP, and HVLDL visits.
- Serve as MUSE Champion to oversee changes in patient acuity and required daily visits.
- Actively participate in Quality Assessment and Performance Improvement (QAPI) activities.
- Participate in the development and update of patient care policies and emergency procedures.
- Make post-admissions calls if directed by leadership.
- Document in patient clinical records as required and demonstrate proficiency with documentation standards.
- Participate in care delivery, including admissions, recertification visits, routine visits, and discharge visits.
- Manage a caseload of patients to support census changes.
- Participate in after-hours shifts, including primary and backup on-call, and perform patient admissions and visits as needed.
- Make visits to hospitals or customers for additional educational support.
- Perform RN Case Manager responsibilities as needed, including carrying a caseload due to patient census fluctuations.
- Perform other duties as assigned.
Requirements
- Active and unencumbered Registered Nurse License.
- One (1) year of Hospice experience.
- Valid driver’s license and auto liability insurance.
- Current CPR certification.
Skills
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, and external parties. Strong written and verbal communication skills.
- Ability to understand, read, write, and speak English.

Physical Demands and Work Environment
The role requires regular standing, walking, and manual dexterity, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Exposure to bodily fluids, infectious diseases, and conditions typical to a healthcare setting is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
