About the role

As a Hospice RN Patient Care Coordinator, you will play a crucial role in supporting patients and their families during their final days. You will coordinate care plans, manage patient transitions, and ensure compliance with hospice standards.

Responsibilities

• Develop and implement comprehensive care plans for hospice patients.
• Coordinate with interdisciplinary team members to ensure seamless care delivery.
• Manage patient transitions between facilities and coordinate with other healthcare providers.
• Ensure compliance with hospice standards and regulatory requirements.
• Provide emotional and spiritual support to patients and families.

Requirements

• Bachelor’s degree in Nursing or related field required.
• Current RN license in good standing.
• Experience in hospice care or palliative care preferred.
• Strong communication and organizational skills.
• Ability to handle sensitive and confidential information.

Pay
