Patient Care Coordinator Registered Nurse (RN) -Hospice

About the role

Join our team as a Hospice Patient Care Coordinator (PCC) and make a meaningful difference during one of life’s most important journeys. In this role you’ll support patients and their families, collaborate with a compassionate clinical team, and help ensure high‑quality, patient‑centered hospice care.

Responsibilities

• Maintain and actively promote effective communication with residents and family members/personal representatives.
• Maintain a positive image of Marquardt in the community in alignment with mission, vision, and values.
• Supervise and direct care of nursing personnel including RNs, LPNs, and Hospice Aids.
• Support/triage patient care visits and clinical questions; serve as RN Case Manager as needed.
• Teach appropriate interventions, medications, effects, and side effects to patients, caregivers, volunteers, and others as appropriate.
• Lead, collaborate, and participate in interdisciplinary team (IDT) meetings to establish, review, and revise the plan of care for each patient.
• In conjunction with the Director, lead the assigned hospice region to achieve organizational annual goals and objectives and assure implementation as scheduled.
• Support the hospice intake and referral process to ensure timely and appropriate response to inquiries and admissions.
• Work with providers and the hospice team to gather and review patient‑specific medical information to ensure appropriate patient eligibility.
• Participate in the Quality Assessment Performance Improvement Program (QAPI) as requested.
• Ensure corporate policies and procedures are administered effectively and fairly.
• Foster collaborative relationships with area partners and manage the admissions processes.
• Provide backup and support to other hospice regions and/or PCCs as needed.
• Ensure compliance with all local, state, and federal codes pertaining to agency operations.
• Perform on‑call services as assigned.
• Perform other similar duties as needed.

• Ability to exercise independent judgment, make sound decisions, and exhibit flexibility when needed.

Qualifications

• Current Wisconsin (WI) Registered Nurse license.
• Four to five years of hospice experience.
• Four‑year degree preferred.
• Valid WI driver’s license.

Schedule

Monday – Friday, 8:00 AM to 4:30 PM. Rotating on‑call every 6th weekend.

Pay

$88,400.00 to $105,352.00 yearly (higher pay may be considered depending on qualifications and experience).
