About the role
Join as a Registered Nurse Case Manager in hospice care, responsible for developing and implementing individualized care plans for patients facing end-of-life. Your role includes documenting patient progress, educating families on self-care techniques, and ensuring comprehensive, compassionate support across diverse home settings.
Responsibilities
- Conduct initial evaluations within 48 hours of referral
- Assess patient and family needs regularly
- Coordinate care with the interdisciplinary team
- Document patient progress

Requirements
- Current and unrestricted RN licensure in the practice state
- Valid driver's license, vehicle insurance, and access to reliable transportation
Preferred Qualifications
- At least 1 year of clinical experience
- CPR certification or ability to obtain within 90 days of hire

Compensation
Salary ranges from $75,620 to $113,430 annually, supplemented by benefits including health coverage, incentives, 401(k), and stock options.
