Role Overview
The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care by developing and implementing patient-specific Plans of Care, collaborating with interdisciplinary teams, and evaluating patient needs regularly.
Key Responsibilities
- Conduct initial nursing evaluations to determine hospice eligibility within 48 hours of referral.
- Assess and reassess patients' physical, psychosocial, and environmental needs at least every 15 days.
- Develop, coordinate, and document the care plan, goals, interventions, and patient responses.
- Collaborate with patients, families, physicians, and team members to ensure quality care.
- Educate and supervise patients and families on self-care techniques when appropriate.
- Maintain accurate clinical documentation regarding patient conditions.
Qualifications & Skills
- Current, unrestricted RN license in the practice state.
- CPR certification or ability to obtain within 90 days.
- Valid driver's license, reliable vehicle or access to transportation.
- Ability to work effectively in diverse home settings regardless of background or circumstances.
- Minimum of 1+ years of clinical experience (state-specific requirements may vary).
