Registered Nurse - Hospice

Role Overview

The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care by developing and implementing patient-specific Plans of Care, collaborating with interdisciplinary teams, and evaluating patient needs regularly.

Key Responsibilities

• Conduct initial nursing evaluations to determine hospice eligibility within 48 hours of referral.
• Assess and reassess patients' physical, psychosocial, and environmental needs at least every 15 days.
• Develop, coordinate, and document the care plan, goals, interventions, and patient responses.
• Collaborate with patients, families, physicians, and team members to ensure quality care.
• Educate and supervise patients and families on self-care techniques when appropriate.
• Maintain accurate clinical documentation regarding patient conditions.

Qualifications & Skills

• Current, unrestricted RN license in the practice state.
• CPR certification or ability to obtain within 90 days.
• Valid driver's license, reliable vehicle or access to transportation.
• Ability to work effectively in diverse home settings regardless of background or circumstances.
• Minimum of 1+ years of clinical experience (state-specific requirements may vary).
