Registered Nurse - Hospice

About the role

We are seeking a Registered Nurse Case Manager to deliver comprehensive hospice care, develop personalized Plans of Care, and coordinate treatment strategies that support patients' physical, psychosocial, emotional, and spiritual needs.

Responsibilities

• Conducting initial eligibility evaluations
• Reassessing patient needs at least every 15 days
• Documenting care progress
• Collaborating with patients, families, physicians, and interdisciplinary teams
• Providing education on self-care techniques

Qualifications

• Current, unrestricted RN license in the practice state
• Reliable transportation

Preferred qualifications

• Over one year of clinical experience
• CPR certification
