Registered Nurse - Home Health (Full-Time)

Role Overview

The Home Health Registered Nurse manages and delivers comprehensive home health services using the nursing process, including assessments, interventions, and supportive care to patients in their homes. Responsibilities involve telephone triage, problem solving, patient/caregiver advocacy, and efforts to avoid hospitalization. The nurse collaborates effectively with patients, families, healthcare providers, and community resources to ensure safe and efficient care while maintaining safety in the home environment.

Qualifications and Skills

• Massachusetts/New Hampshire RN Licensure
• Valid driver's license
• At least 1 year of acute medical/surgical nursing experience
• CPR Certification

Key Duties

• Assess patients' physical and psychosocial needs and develop personalized care plans
• Adjust care delivery based on patient learning styles and needs
• Establish goals, complete physician orders, and document care accurately
• Manage medication reconciliation and provide emotional support to patients and families
• Coordinate care, discharge planning, and collaborate with healthcare team members
• Prioritize patient visits and respond promptly to messages and calls
• Maintain knowledge of regulatory standards, third-party payers, and documentation policies
• Ensure effective communication within the home care team, addressing critical needs or crises
