About the role
The RN Complex Care Coordinator assesses and manages care for complex patients, ensuring safe and timely discharges across the continuum. They serve as a clinical and psychosocial resource, coordinating multidisciplinary efforts and engaging patients and families in decision-making.
Responsibilities
- Perform comprehensive assessments of high-risk patients, addressing clinical, psychosocial, and resource needs.
- Develop and implement individualized discharge plans, overcoming barriers and coordinating post-acute care referrals.
- Partner with care teams, community resources, and families to support patient transitions and resource access.
- Document assessments, interventions, and progress accurately in electronic health records.
- Mentor and precept new staff and interns, sharing expertise on complex social and clinical issues.
- Support quality improvement initiatives and comply with organizational policies.
- Participate in interdisciplinary rounds and facilitate family meetings to optimize care outcomes.
Requirements
- Associates or Bachelor’s Nursing degree (BLS and RN licensure required).
- Minimum 3 years healthcare experience, with at least 2 years in care coordination within acute care.
- Strong communication, assessment, organizational, and problem-solving skills.
- Knowledge of regulations, community resources, and multidisciplinary collaboration.
- Ability to handle stressful situations and work effectively in fast-paced environments.
