Regional Traveler - HouseCalls Nurse Practitioner

About the role

Home-based Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) conducting in-home health assessments for Medicare and other plan members.

Responsibilities

• Perform comprehensive assessments, including medical history, physical exams, screenings, and point-of-care tests.
• Identify and document diagnoses.
• Communicate findings to primary care providers.
• Recognize urgent situations and take appropriate action.
• Educate members on health management.
• Address social determinants of health with referrals.

Requirements

• Active certification (ANCC/AANP).
• Unrestricted licensure as a Nurse Practitioner or Physician Assistant.
• 1+ years of clinical experience.
• Valid driver’s license.
• Ability to navigate home environments.

Skills & Preferences

• Proficiency with electronic medical records.
• Ability to transport equipment.
• Strong communication skills with diverse populations.

Schedule

Flexible hours between 8:00 AM and 8:00 PM, with no on-call, weekend, or holiday shifts. Regional travel required (85%).
