Nurse Practitioner, HouseCalls Statewide Traveler - HI

Role Overview

This position involves conducting in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care and care coordination. It requires approximately 85% travel across Hawaii, with all expenses covered, and offers a flexible schedule between 8am-8pm, with no on-call, weekends, or holidays.

Key Responsibilities

• Perform comprehensive assessments including medical history, medication reconciliation, vital signs, and screenings.
• Document diagnoses and communicate findings to primary care providers.
• Recognize urgent situations and intervene appropriately.
• Educate patients on health conditions and medications, addressing social determinants of health and providing referrals.

Qualifications & Skills

• Active licensure and certification (e.g., ANCC, AANP) for Nurse Practitioners or Physician Assistants.
• Valid driver's license and reliable transportation.
• Ability to navigate home environments and perform physical assessments.

PREFERRED EXPERIENCE:

• Experience in family, geriatric, or home health care.
• Proficiency with electronic medical records.
• Strong communication skills.
