Role Overview
This position involves conducting in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care and care coordination. It requires approximately 85% travel across Hawaii, with all expenses covered, and offers a flexible schedule between 8am-8pm, with no on-call, weekends, or holidays.
Key Responsibilities
- Perform comprehensive assessments including medical history, medication reconciliation, vital signs, and screenings.
- Document diagnoses and communicate findings to primary care providers.
- Recognize urgent situations and intervene appropriately.
- Educate patients on health conditions and medications, addressing social determinants of health and providing referrals.
Qualifications & Skills
- Active licensure and certification (e.g., ANCC, AANP) for Nurse Practitioners or Physician Assistants.
- Valid driver's license and reliable transportation.
- Ability to navigate home environments and perform physical assessments.
PREFERRED EXPERIENCE:
- Experience in family, geriatric, or home health care.
- Proficiency with electronic medical records.
- Strong communication skills.
