About the role
Join a patient-centered, field-based clinical role focused on in-home health assessments for Medicare Advantage members.
Responsibilities
- Conducting comprehensive assessments: medical history, medication reconciliation, vital signs, physical exams, and screenings.
- Documenting diagnoses and collaborating with primary care providers to address care gaps.
- Recognizing urgent situations and providing appropriate interventions.
- Educating patients on health conditions, medications, and social determinants of health, with referrals as needed.
Requirements
- Active certification (ANCC or AANP) in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
- Unrestricted licensure in practicing state without disciplinary actions.
- Minimum of 1 year clinical experience, preferably in family, geriatric, or home health care.
- Valid driver’s license and ability to navigate diverse home environments.
Preferred Skills
- Proficiency with electronic medical records and technology.
- Ability to transport equipment and navigate stairs during home visits.
- Strong communication skills with geriatric or Medicare populations.
