Role Overview
As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct comprehensive in-home health assessments for Medicare Advantage and other members, focusing on preventive care, education, and care gap closure. Your responsibilities include reviewing medical history, performing physical exams, documenting diagnoses, communicating with primary care providers, recognizing urgent situations, and addressing social determinants of health.
Key Responsibilities
- Perform in-home assessments and screenings
- Identify and document health conditions
- Coordinate with primary care providers
- Educate patients on health management
- Address urgent/emergent issues and social needs
Qualifications & Skills
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- Unrestricted licensure in Colorado
- At least 1 year of clinical experience
- Valid driver's license and reliable transportation
- Ability to navigate home environments and perform physical assessments
Preferred Skills
- Proficiency with electronic medical records and technology
- Ability to transport equipment and navigate stairs
- Strong communication skills with geriatric or Medicare populations
