Role Overview

The HouseCalls Advanced Practice Clinician conducts comprehensive in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care, education, and care gap closure.

Responsibilities

• Perform medical history reviews, physical exams, screenings, and point-of-care tests
• Document diagnoses
• Communicate with primary care providers
• Identify urgent situations
• Educate members on health management
• Address social determinants and refer as needed

Qualifications & Skills

• Active, unrestricted licensure and certification (NP or PA)
• Valid driver's license, reliable transportation
• Ability to navigate varied home environments
• Prior clinical work with geriatric or Medicare populations preferred
• Proficiency with electronic records preferred
• Ability to lift equipment and navigate stairs required

Compensation & Benefits

• Total cash compensation ranges from $109,500 to $164,000, including base pay, bonuses, and incentives
