Referral Coordinator

Duties & Responsibilities: Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safetyEnsure complete and accurate registration, including patient demographics and current insurance informationMust enter and update referral data in electronic system as appropriateAssemble information concerning patient's clinical background and referral needsContact review organizations and insurance companies to ensure prior approval requirements are metPresent necessary medical information such as history, diagnosis and prognosisProvide specific medical information to financial services to maximize reimbursement to the hospital and physiciansReview details and expectations about the referral with patientsAssist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance)Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providersEnsure that referrals are addressed in a timely mannerEnsure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health.' Required Skills/Abilities: Excellent verbal and written communication skillsExcellent customer service skillsExcellent organizational skills and attention to detailProficient with Microsoft Office Suite or related softwareBilingual (English/Spanish) Education and Experience: High school diploma or equivalent required. Minimum requirement of one year work experience with referrals, or related medical office experienceExperience with EMR systems is preferred Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Must be able to lift up to 10 pounds at times.
