About the Role
The Certified Recovery Peer Advocate I (CRPA I) functions as a member of an individual's treatment team to assist with the outreach, support, and coordination of care for eligible service recipients. The CRPA I will serve as a role model and/or health coach for clients, promoting the agency and its programs in the community. This role involves working with agency personnel and outside agencies to provide education on drug and alcohol use prevention services.
Requirements
- High School Diploma or GED
- Provisional certification (CRPA-P) upon hire and receipt of full certification (CRPA) within one year
- Knowledge of medication-assisted treatment, opiate overdose prevention, and substance use disorders treatment and recovery services
- Relevant life experience: personal experience living with a disability, history of substance use, or social/emotional challenge, and willingness to draw upon this experience to serve as a role model
- Knowledge of community resources and community outreach preferred
Responsibilities
- Provide outreach to clients to facilitate engagement in services and offer additional supports for participation in Medication-Assisted Treatment and mobile services
- Provide health coaching and recovery support on issues such as self-advocacy, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities
- Provide transportation for and accompany clients to medical and/or other appointments as needed
- Deliver concrete services to clients, such as benefits/enrollments, per program needs
- Maintain organized, timely, and accurate documents, records, statistics, and related reports
- Coordinate care planning with other service providers to ensure goal-directed, collaborative care, including care transitions
- Act as a resource/consultant to team members on substance abuse issues
- Conduct telephonic and face-to-face outreach, engagement, and service planning in the field
- Link clients to community services, including medical, behavioral, residential, entitlement, and other needed services
- Monitor overall service delivery to ensure coordination and continuity; advocate with service providers as needed
- Provide crisis intervention and follow-up
- Act as a liaison between the agency and the communities it serves
- Represent the agency professionally and knowledgeably
- Promote the agency through information and education about its services
- Conduct direct outreach to courts, attorneys, hospitals, probation, parole, DSS, CPS, DDP, PCPs, schools, and other community groups
- Report and work under the direction of the director(s)
- Represent the agency at community events such as NARCAN trainings, fairs, and forums
- Document and report on community contacts with the director(s)
- Drive company vehicles as needed
- Facilitate data sharing with clients to support partnered decision-making and keep them informed of progress
- Complete QI Training during onboarding and participate in refresher training as needed
- Cultivate a culture of inclusion that respects individual strengths, views, and experiences
- Adhere to all safety protocols and procedures to ensure a safe working environment
- Perform other duties as assigned
