Rapid Rehousing & Homeless Prevention Case Manager (SSVF)- Worcester, MA

About the role

The Rapid Rehousing & Homeless Prevention Case Manager (SSVF) will provide direct services to individuals experiencing homelessness in Worcester, MA.

Responsibilities

• Assess client needs and develop individualized service plans
• Coordinate access to housing, healthcare, and social services
• Provide case management and support to clients
• Document progress and outcomes in client files
• Maintain confidentiality and adhere to agency policies and procedures

Requirements

• Bachelor’s degree in Social Work, Public Health, or related field
• At least 2 years of relevant experience in homelessness services
• Licenses/certifications preferred but not required

Qualifications

• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite

Skills

• Knowledge of local resources and services for homeless populations
• Experience with trauma-informed care
• Excellent organizational and time management skills
