Key Responsibilities
- Provide skill-building training in the home or out in the community: one-on-one skills sessions at home, at the office, or in the community relating to goals and objectives using evidence-based practices and curriculums.
- Support and crisis/emergency services for the child/family through face-to-face contact, phone contact, or availability by an on-call system (all time spent on crisis assessment and planning, crisis linkage and follow-up, and stabilization must be documented as crisis hours).
- Maintain all client documentation in the electronic record and write progress notes adhering to requirements set by the Texas Administrative Code.
- Utilize effective listening skills to establish rapport with clients, family members, and other individuals close to the client; provide emotional support and encouragement and engage family members in the service process.
- Maintains records by regulatory/agency standards. Enters data in a clear, accurate, and timely manner. Also, promptly submits documentation needed for the tracking of output/outcome measures. May be asked to assist in record review or qualitative review processes.
- Keep the family and team members informed on progress toward achieving Service Plan goals.
- Aid clients with daily living tasks such as behavior management, counseling services to the client, budget management, independent transportation assistance, time management skills, and other tasks to promote independent living.
- Collaborate with court/probation, tribal/community agencies, families, and other natural support systems to ensure clients’ needs are met. Develop a strong working relationship with community partners.
- Aid clients in practicing effective communication, problem-solving, and anger management techniques in their daily lives.
- Participate in staff meetings, monthly training, company programs, and professional development training as required.
Requirements
- Must have a Bachelors' Degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
- Applicants applying for Case Manager must have Master's Degree.
- All applicants must have at least (1) year of paid experience.
- ANSA/CANSA Certified Preferred.
