About the role
The Pima County Health Department is seeking a compassionate and motivated Peer Navigator to support individuals and families impacted by substance use disorders. Key responsibilities include providing peer support, guiding clients through healthcare goals, collaborating with care teams, leading community outreach, and advocating for clients.
Responsibilities
- Maintain written and electronic client records documenting client history and services provided.
- Collaborate with care teams to achieve healthcare goals for program participants.
- Make presentations to community interest groups on program-related topics.
- Assist public health professionals in health navigation and reducing disparities in the community.
- Refer to internal or external case management services when other issues are identified.
- Act as a department representative and subject matter expert to assigned programs.
- Develop and maintain effective working relationships and coordinate program activities with various stakeholders.
