About the role
This is an exempt (salaried) position. Pay rates are based on education, skill, experience level and internal equity. This is a Grant Funded Position through December 31, 2026, with renewal options based on available funding.
Responsibilities
- Provide professional-level Care management services for various populations.
- Works in a variety of settings coordinating care needs across the healthcare continuum.
- Assist pregnant clients in accessing health care and support services to reduce infant mortality through improved pregnancy outcomes.
- Affirm the provision of comprehensive preventive health services to pregnant women and their infants.
- Aid pregnant women to meet other priority needs that affect their well-being and that of their families.
- Collaborate closely with the client, the client’s support system, medical providers as well as other community service providers.
- Maintain case history records and prepare reports.
- Serve on program-specific policymaking committees within the department, assist in communication with community partners and assist client groups by lobbying for solutions to problems.
- Utilize professional training and the principles and practices of public health to provide health care services to all populations served.
- Participate in MCHD response activities as needed.
Requirements
- Experience: Minimum of one year of public health, healthcare and/or regulatory experience.
- Education: Social Worker: MSW from an accredited school of social work; BSW from an accredited school of social work and one year of social work experience. Nurse: BSN from an accredited nursing education program; ADN from an accredited nursing education program.
- Combination of relevant education and relevant experience accepted: N/A.
- Licenses and Certifications: Current certification in Basic Life Support for Healthcare Providers (CPR & AED) by the American Heart Association that is valid for at least 90 days after hire. Requires a valid North Carolina or South Carolina Drivers License. Requires County Driving Privileges. Current license to practice as a Registered Nurse (RN) in NC issued by the NC Board of Nursing or another Nurse Licensure Compact State.
- Preferred Qualifications: BSN with one (1) year of relevant nursing care management experience preferred. ADN with three (3) years of relevant nursing care management experience preferred.
Qualifications
- Knowledge: Knowledge of social services procedures, practices and methods. Knowledge of social services related federal, state and local laws, rules and regulations relating to provision of public assistance, and social welfare. Knowledge of care management methods and techniques. Knowledge of services offered by the county and other local public and private community-based service agencies. Knowledge of medical and legal terminology. Knowledge of principles of public health and associated laws.
- Skills: Prioritizing work. Managing and executing multiple tasks. Maintaining various confidential records. Evaluating records to ensure accreditation standards are met. Using computers and related software applications. Exercising judgment and discretion. Using professional training and the principles and practices of public health to provide health care services to all populations served.

Computer Skills: Proficient in various computer applications including Microsoft Office Suite.
