Psychiatric Social Worker - Mobile Crisis Team
About the role
We are seeking a Psychiatric Social Worker to provide client-centered behavioral health care across various settings, including outpatient clinics, homes, and community outreach. The role involves screening referrals, conducting psychosocial evaluations, and developing short-term service plans. Providing counseling, care management, and facilitating referrals for long-term treatment. Maintaining detailed case records and encouraging clients to accept mental health services. Participating in interdisciplinary team meetings, coordinating with community agencies, and offering outreach services. Serving as a resource on psychiatric topics and contributing to program development and training. Monitoring mental health status, especially among seniors, and supporting crisis intervention teams as needed. Contributing to community programs and policy development.
Qualifications
- Current NYS Licensed Master Social Worker registration
- Master's Degree in Social Work from an accredited program
- Minimum of one year experience in a healthcare setting
- Valid driver’s license may be required based on operational needs
- Bilingual skills may be preferred
