Responsibilities

• Deliver outreach services

Qualifications

• Licensed Master Social Worker registration in New York State required
• Minimum one year of social work experience in healthcare setting
• Graduate degree in social work from an accredited program
• Valid driver’s license may be required
• Bilingual skills advantageous
• Ability to access client residences, including walking stairs
