About the role
We are seeking a Psychiatric Social Worker to provide psychosocial services to mentally ill patients in the community.
Responsibilities
- Screen telephone referrals
- Conduct psychosocial evaluations and assessments of patients and families
- Develop and implement short-term service plans
- Provide counseling, care management, and referrals for long-term treatment
- Maintain case records and histories
- Encourage clients to accept mental health services
- Participate in interdisciplinary team meetings and case conferences
- Coordinate linkages with community agencies and mental health providers
- Offer outreach services to at-risk individuals in their homes
- Serve as a resource for psychiatric nursing and training
- Monitor mental status of senior clients
- Provide crisis intervention and support as part of a flexible response team (for Mobile Crisis - Adult FLOAT Team)
- Contribute to policy development, community education, and special projects
Qualifications
- Licensed Master Social Worker (LMSW) registration in New York State
- Master’s Degree in Social Work from an accredited program
- Minimum of one year healthcare social work experience
- Valid driver’s license and NPI number may be required
- Bilingual skills may be advantageous
