Responsibilities
- Screen telephone referrals
- Conduct psychosocial evaluations
- Develop short-term service plans
- Provide counseling and care management
- Facilitate referrals to long-term mental health and social services
- Coordinate with interdisciplinary teams, community agencies, and clients to ensure continuity of care and support recovery
- Conduct outreach to at-risk individuals
- Participate in community programs
- Provide clinical supervision (as applicable)
- Participate in policy development
- Perform crisis response duties
Requirements
- Current NYS Licensed Master Social Worker license
- At least one year of healthcare social work experience
- Flexibility and strong coordination skills
