About the Role

The Psychiatric Mental Health NP Supervisor plays a critical role in supporting integrated behavioral health care within a collaborative care model, focusing on high-risk populations including individuals with serious mental illness (SMI), substance use disorders (SUD), or complex mental health needs impacting overall health outcomes and care utilization. This role provides psychiatric expertise to behavioral health care managers, primary care providers, and interdisciplinary teams, guiding treatment planning, monitoring patient progress via registries, and ensuring timely care adjustments. The supervisor contributes to system design, quality improvement initiatives, and program development aimed at enhancing behavioral health integration, reducing readmissions, and closing care gaps across the continuum.

Responsibilities

  • Direct evaluation of patients with significant diagnostic or therapeutic challenges, identified in collaboration with the patient’s behavioral health care manager and/or primary care provider. The target population includes individuals with serious mental illness (SMI), substance use disorder (SUD), or unstable mental illness contributing to cost and utilization patterns. Consultations may be provided in person or via telemedicine (video or telephone).
  • Be well-versed in integrated behavioral health models and collaborative care.
  • Support registry management, outreach to membership, and closure of care gaps.
  • Provide regularly scheduled consultation to behavioral health care managers and interdisciplinary teams on a weekly basis.
  • Build a behavioral health program aligned with organizational KPIs, particularly in managing transition episodes and readmissions.
  • Conduct case reviews for participants who are new to the caseload, not improving as expected, in a transition episode, or determined to be high risk. Reviews may be conducted by telephone, video, or in person.
  • Work with behavioral health care managers to oversee the status and population health of the assigned patient caseload.
  • Use a registry to track all assigned patients, prioritize them for case review and treatment adjustment, and recommend treatment adjustments within 24 hours of consultation.
  • Advise on treatment for participants needing more intensive or specialized mental health care and support them until connected to appropriate community resources.
  • Communicate the plan of care to primary care providers both within and outside the organization.
  • Provide occasional telephone consultation to medical providers for complex patients or challenging situations, responding within one business day or two hours for urgent calls.
  • Deliver in-service training for primary care-based providers and staff on recognizing and treating behavioral health conditions in primary care.
  • Apply programmatic development skills and interest in system design, maintaining a product lens through the delivery of care and redesigning services around participant needs.
  • Demonstrate interest in quality improvement, measurement, and the ability to articulate the program's key performance indicators and tracking to targets.

Requirements

  • Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.
  • Licensed to practice as a Psychiatric Nurse Practitioner in NY.
  • Licensed to practice as a Registered Nurse in NY.
  • DEA Certification required.
  • AANP or ANCC Board Certification preferred.
  • Two (2) years of clinical nursing practice in home care, care management, or a related field preferred.
  • One (1) year of experience working with a frail or elderly population, or appropriate training upon hiring if less than one year of experience.
  • Effective oral and written communication and interpersonal skills.
  • High-level analytical and research skills.
  • Ability to manage multiple projects and meet deadlines.
  • Basic computer skills in Microsoft Office.
  • Willingness to travel to different boroughs.
  • Understanding and experience in value-based settings, managed care, or digital health startups preferred.
  • Leadership skills and ability to lead teams through influence.

Physical Requirements

  • Standing for up to 6 hours a day.
  • Sitting/stationary positions for up to 6-8 hours a day for consecutive periods.
  • Lifting, pushing, or pulling up to 50 pounds of equipment, supplies, or patients safely using OSHA guidelines.
  • Bending or squatting to care for patients safely and use medical supplies.
  • Maneuvering stairs, walking between sites/locations, and accessing work areas throughout the day.
  • Demonstrating agility and fine motor skills to operate equipment, devices, instruments, and tools (e.g., typing, using medical supplies).
  • Visual ability to assess patients, read orders, and document information accurately.
  • Auditory ability to listen attentively and document information from patients, providers, and others with accuracy, as well as speak clearly.
  • Cognitive ability to demonstrate good decision-making, reasoning, and analysis.

Pay

Salary Range: $175,000.00 - $185,000.00

8:00 AM – 5:00 PM
Weekly Hours: 40
